Job Description


Provides direct and indirect patient care services that meet the psychosocial, physical and general aspects of patient care. Meets the communication needs of patients and families. Knowledgeable of a wide range of disorders; cognitive, emotional, developmental, social and behavioral. Provides care that reflects initiative and responsibility indicative of professional expectations, under the supervision of a registered nurse. Performs crisis intervention. Participates in the unit’s performance improvement and continuous quality improvement (CQI) activities.

Job Duties:

• Performs patient care responsibilities considering specific needs to the standard of care for the patient’s age and clinical condition.
• Demonstrates the ability, under the direct care of an RN, to perform nursing assessments on all patients of all ages and reassessments as per policy.
• Ability to adequately assess and reassess pain. Utilizes appropriate pain management techniques. Educates the patient and family regarding pain management.
• Demonstrates knowledge of medications, including psychotropic drugs and their correct administration based on the patient’s age and clinical condition.
• Follows the six medication rights and reduces the potential for medication errors.
• Adheres to the patient plans of care; family is included when appropriate.
• Demonstrates ability to revise the plan of care as indicated by the patient’s response to treatment and evaluate overall plan daily for effectiveness.
• Continuously observes patient’s behavior, mental status and activities.
• Demonstrates ability to perform treatments and provide services to level of licensure.
• Supports and maintains a culture of safety and quality.
• Communicates appropriately and clearly to team leader, co-workers, physicians and patients.
• Concurs with members of the treatment team for the benefit of the patient.
• Consults other departments, as appropriate, to provide for an interdisciplinary approach to the patient’s needs.
• Concurs with members of the treatment team for the benefit of the patient.
• Provides nursing care appropriate to the mental and physical condition of the patient.
• Performs all aspects of patient care in an environment that optimizes patient safety and reduces the likelihood of medical/health care errors.
• Demonstrates the ability to perform crisis intervention.
• Documentation meets current standards and policies and is completed within shift worked.
• Assist with direct patient care when needed.
• Maintains and operates equipment safely and correctly.
• Treats patients and families with respect and dignity; provides emotional support.
• Provides education to the patient and family concerning plans of care, compliance with the plan of care, medications and their side effects.
• Functions as a patient and family advocate at all times.
• Maintains a good working relationship within the department as well as other departments/units.
• Demonstrates an ability to be flexible, organized and function under stressful situations.
• Coordinates and supervisors patient care under the direct supervision of a registered nurse.
• Response to the patients is appropriate in distressful situations.
• Performs other duties as assigned within the LPN/LVN scope of practice.
Professional Requirements:

• Meets dress code standards; appearance is professional, neat and clean.
• Completes annual educational requirements.
• Maintains regulatory requirements and ensures patient confidentiality at all times.
• Delegates task as appropriate using the nursing process as a guide.
• Reports to work on time and as scheduled.
• Wears identification while on duty.
• Attends in-services and returns in a timely fashion to the unit.
• Attends required staff meetings and provides feedback to improve services.
• Actively participates in performance improvement and continuous quality (CQI) activities.
• Complies with all organization policies, procedures and staff handbook.
• Completes all documentation as outlines in policy and procedures.
Regulatory Requirements:

• Current state Licensed Practical Nurse Licensure.
• Current BCLS certification.
• CPI certification preferred within (60) days of hire.
• Current CPR/First aide certification
