Responsibilities:

 Provide rehabilitative services to patients in the home.

 Complete Occupational Therapy evaluations, treatment and follow-up services, and discharge planning.

 Work as part of a team.

 Develop a therapy treatment plan to improve and restore functions that have been impaired and to achieve established goals.

Requirements:

 Professional License (Occupational Therapy)

 Valid CPR card

 Valid Drivers license

 Valid Car Insurance

 Two year's of home health experience

 Comfortable in using online Electronic Medical Record (EMR) system for documenting We use TherapyBoss.

Job Types: Part-time, Contract

Salary: $70.00 - $75.00 per hour
